Misc: Customer Satisfaction Survey?

Dear Colleagues:

Now that we have been in business for 1 year (doing both peptide synthesis and
protein sequencing), we would like to put together a Rsatisfaction surveyS to
send to our clients.

If any of you have done this I would appreciate any suggestions you have.

If someone could actually send me a copy of their survey (either by email:
ettlingr@musc.edu or Fax: (843) 792-1264) that, would be extremely helpful.

How about some thoughts on whether it is better to
a) send a mass mailing once a year
b) send the survey ~1 month after the work is done
c) attach a questionnaire to the back of every report ?

It seems to me that both b) and c) might get annoying to major users but a)
gives no immediate feedback and might unnecessarily delay your awareness of a
problem ?

What percentage of questionnaires actually get completed and sent back ?
