I also have had this problem (although infrequently and to a lesser degree), but it
is always the same client's template/primer. First comes nothing, or a smear, then
perfectly normal sequence! All the other samples are normal on the same gel. If all
your samples are coming from the same lab, it may be a fault of the DNA vector or
the primer used.

I hope this helps gives you a clue to the problem.
